Case Management is a process, in which the Case Manager is committed to go the distance by offering an ongoing individualised ‘client-centred’ service through a process of assessment, formation, facilitation and monitoring. Case Management is provided to both adults and children with ongoing complex needs as a result of acquired brain injury, spinal and orthopaedic injuries.

A Case Manager confidently assists the client in navigating through a complex array of possible paths as part of their recovery. Identifying and coordinating the expertise and support of professionals and family members, to ensure personalised quality of care and rehabilitation.

What are the responsibilities of a Case Manager?

The responsibilities of a Case Manager are as varied as the clients themselves. The following illustrates a selection of areas in which a Case Manager can become involved:

  • Supporting a client to identify their goals and monitor their progress.
  • Sourcing and collaboratively working with appropriate therapists.
  • Developing Care Packages and commissioning Rehabilitation Programmes.
  • Collaboratively working with both statutory services and those in the private sector.
  • Assisting with aspects of housing and adaptations.
  • Sourcing specialist equipment.
  • Assisting with areas of education and vocational activities.
  • Impartially advocating on behalf of the client.

What service does a Case Manager provide?

A Case Manager will assess the needs of the client and outline objective clinical recommendations. They offer a personalised approach that develops a bespoke rehabilitation programme which undergoes ongoing monitoring and review.

The aim of the service is to offer individuals opportunity to improve their quality of life, provide strategies for minimising the effects of injury, maximise potential and work towards clearly defined goals.

The service works to empower each client, through enabling and educating all those involved, providing them with a voice in the delivery of their care and ensuring particular attention to their specific needs.

Case Management services are provided to clients for as long as they require. For some this might be a couple of years as part of the litigation process, whilst others might require a level of intervention over the course of their lifetime.
